Concord Health and Rehabilitation Center offers 63 private rooms for our guests, who may require short term rehabilitation, specialized dementia care, ventilator care, tracheostomy care, respiratory care, and long term care. Whether you need rehabilitation services following a recent illness, our staff of trained professionals are here to help you meet your goals so you can return home as quickly as possible so you can return to doing the things you love. We also have trained Respiratory Therapists and nurses who specialize in ventilator and tracheostomy care for our guests who may require a higher level of care. We also have trained nursing staff and activities staff who provide care and services to those who have Alzheimer's disease and other forms of dementia.

Concord Health and Rehabilitation Center is also excited to announce a new program known as the "Wishing Program." The "Wishing Program" is designed to grant small wishes to our guests during their stay at our nursing facility. The facility representative completes a personalized questionnaire with each new guest and then will begin the wish-granting process with our guest and their family. We want to use this as an opportunity to make each guest stay with us a memorable experience.
