Portsmouth Parade Welcomes Health and Rehab Center

Concord Health & Rehabilitation Center took part in the Portsmouth River Days Parade! So much fun for our staff and residents!

We love our community and how we can connect together in Wheelersburg and Portsmouth. Concord and its parent company, Foundations Health Solutions, strive to follow the vision of founder Brian Colleran by meeting the needs of residents in Akron, Cincinnati, Cleveland, Columbus, Dayton, and anywhere in-between with a wide range of services within our centers, including post-hospital stay, skilled nursing, inpatient rehabilitation, long-term care, memory care, and ventilator services.
