Wheelersburg's only 4-Star Rated Nursing Facility

Concord Health and Rehabilitation Center, in Wheelersburg, Ohio, is excited to announce that the facility has recently improved to 4-Star Overall Rated Nursing Home through The Centers for Medicare & Medicaid Services star rating system (located on
Medicare.gov). The Centers for Medicare & Medicaid Services rates facilities based on the following metrics: overall rating, health inspections, staffing, and quality measures. The residents and employees are excited to announce this achievement to our families and the community that we serve.
Four years ago, Concord set out to improve its CMS Star Rating. At the time, Concord was rated as a 1-star overall nursing facility. The team at Concord decided that improving resident care, resident outcomes, and their 5-Star Rating had to be a priority for our residents and to the community we serve because Concord strives to be the facility of choice in our community. The team at Concord focused on improving staffing levels, quality measures, outcomes for our residents, and health inspection results conducted by the State of Ohio to improve the quality of life for our residents. Since that time, Concord has made measured and quantifiable progress in improving in all these areas for our residents in helping achieve the 4-star rating.
The improvement has not happened overnight and has not been easy. It has taken a lot of hard work from all staff members in improving its overall rating to 4-stars. Concord has improved its Quality Measures to a 4-star rating. It has also improved its health inspections rating to a 4-star rating as well. Even with this improvement, the team at Concord is ready to continue improving its facility for residents and community. Although achieving a 4-star rating is a great accomplishment, the team at Concord is not finished trying to improve its star rating and resident outcomes. The next challenge that the team at Concord has its eyes on is moving the facility to a 5-star rated facility, and they are on their way in trying to achieve this milestone.
